For any person wanting to participate in any Humboldt Light Opera Company’s (HLOC) activities, each participant or parent or guardian of a participant agrees that:

The term participant means the individual participating or the parent of a participant or the legal guardian of a participant.

Participant consents to participate in HLOC activities.

Participant warrants that participant is in good health and is able to participate in HLOC activities for which participant is registered.

Participant agrees to take responsibility for participant’s safety during any HLOC activities.

Participant agrees that participant shall hold HLOC, its directors, officers, employees, and agents, free from all liability on behalf of the participant whether caused by negligence or otherwise, during any and all HLOC activities and further agrees not to sue HLOC, its directors, officers, employees, and agents.

Participant assumes full responsibility for all risks, including risk of bodily injury, death or property damage to the participant due to the negligence of HLOC, its directors, officers, employees, and agents, or otherwise while in, about or upon the premises used by HLOC and/or while using the premises or any facilities or equipment hereon.

Should there be a significant uptick in Covid, we reserve the right to resume masking in classes and use of hand sanitizer. You will be informed of any chances via email.

Participant has read and voluntarily signs this release and waiver of liability and indemnity agreement, which constitutes the entire agreement as above.
